Sunrise provides comprehensive primary care services for northeastern Colorado in Weld and Larimer Counties regardless of a patient’s ability to pay, including patient navigation and clinical services for breast, colon, and cervical cancer screening. Sunrise’s network of clinics serves as the safety net for the uninsured and underinsured in our service area. Our team of Social Workers, Case Managers, Referral Coordinators, and the Care Coordination Manager collaborate with clinical providers and the MA team to provide individual patient navigation and case management services that move the patient from initial identification and eligibility screening to direct breast and cervical cancer screening and then to a referral network for additional services, diagnostic screening, and treatment, if needed. Our goal is to increase equity for those who are least likely to have access to cancer screening, identification, and treatment – especially Hispanic, low-income, and uninsured populations, among many other vulnerable populations.
